Complications may occur due to various factors including IOL design, improper loading of the IOL or poor surgical technique. In this video, we will show some unforeseen problems during IOL implantation and steps to be taken to prevent these problems to ensure a successful outcome for the patient and the surgeon. FF2-42 Anterior and Posterior Capsular Fibrosis - Managing the enigma!!! Chirag CHHEDA India We present a case of cataract surgery in a hard brown mature cataract with anterior and posterior dense capsular fibrosis. We would like to highlight the importance of performing an equatorial capsulorhexis, a pacman configuration technique of nucleus delivery through a small 5mm incision and posterior capsular plaque peeling. Also, by converting a topical phacoemulsification surgery to a manual small incision cataract surgery in such a complex case in time we can provide excellent visual outcomes to our patients. From a preoperative vision of hand-movements our patient achieved a visual acuity of 6/9 on post operative day 1. During phacoemulsification, low fluidics settings, careful nuclear disassembly, and controlled phaco power minimize stress on the iris. The video highlights cortical cleanup techniques, intraocular lens (IOL) implantation, and intraperative considerations, ensuring optimal visual outcomes while reducing the risk of complications such as iris trauma, miosis, or postoperative inflammation. FF2-44 Phaco in Eyes With Discontinuous Rhexis…the Ten Commandments to Follow Bharathi MEGUR India Performing phacoemulsification in eyes with discontinuous rhexis is fraught with danger. There is a higher risk of a posterior capsular tear and nucleus drop when phacoemulsification is performed in eyes with compromised rhexis. In this teaching video we are highlighting the ten commandments that we follow routinely in our practice when the rhexis is discontinous. By following these principles we can ensure safer and better outcomes while performing phacoemulsification in these eyes. FF2-45 Stratified Phacoemulsification: Unveiling the novel separation technique Rohit OM PARKASH India Surgical outcomes in challenging case scenarios is compromised. Video with graphic animations will showcase how stratified separation of nuclear layers help in improving patient outcomes in challenging situations. Video will show how our technique helps in challenging case scenarios like Morgagnian cataracts, posterior polar cataracts, hard cataracts, zonulopathy, last nuclear fragment, IFIS prevention and patients with compromised endothelial cell count. FF2-46 Dropped It! And Dropped Again!!??? Ankit SHAH India Nucleus and IOL drop are rare but serious complications of cataract surgery. This video will highlight the comprehensive way to manage this complication and how to handle the case with 3port pars plana vitrectomy and also discuss about both complication happened in a single case yet how best we can manage and how to put IOL all in same sitting of the cataract surgery case covering all aspects of intraocular lens options in such a case.
