FF1-13 Bilateral Toric IOL Subluxation in a High Myope Following Cataract Surgery

We report a rare case of bilateral biconvex toric intraocular lens (IOL) subluxation in a high myopic patient following uneventful phacoemulsification cataract surgery. A 37-year-old male with high myopia underwent sequential bilateral cataract surgery two weeks apart. On postoperative day one, subluxation of the toric IOL was observed in both eyes, despite an intact capsular bag and no posterior capsule rupture. The IOLs were successfully repositioned within the capsular bag the following day. Two months post-operatively, the patient's vision is good and remains stable. Ideally, angled three-piece IOls would be preferred in high myopes due to their inherent stability, however toric versions are currently not available. This case highlights the potential for early IOL subluxation in high myopes, even with uneventful surgery. FF1-14 Revisiting Hydrodissection - A little used art Ajay MEHTA India Cortical cleaving hydrodissection (CCH) was presented in detail by Dr Howard Fine three decades ago. But its popularity declined more because of posterior polar cataracts and associated complications due to hydrodissection though they form only a small portion of cataract surgery. This video shows detailed steps of how to perform it (CCH) helping to provide a easily rotatable lens free from all adhesions, avoid the step of cortical aspiration and provide a clean bag at the end of nucleus removal itself. FF1-15 Continuous Curvilinear Capsular Openings - The gateway to perfect phacoemulsification Jeewan S. TITIYAL India Continuous curvilinear capsulorhexis was a landmark event in the field of phacoemulsification, enabling in-the-bag placement of IOLs. It may not be possible to create an ideal capsulorhexis in all cases. Laser capsulotomies allow automated creation of well-centered capsulotomies with optimal morphological characteristics. Different laser devices include femtosecond lasers (FSL), zepto device using a nitinol ring and selective laser capsulotomy (SLC) device based on photocoagulation. We observed comparable morphological outcomes in terms of circularity index, continuity and IOL coverage with different laser devices. Predictability of capsulotomy size was superior with FSL. In intumescent white cataract, SLC achieved complete capsulotomies with slow release of milky fluid after laser delivery; FSL had burst of fluid interrupting laser delivery with residual microadhesions. Visual outcomes were comparable. FF1-16 Markings for Toric IOL Implantation Aman AGRAWAL India This video will show the various preoperative methods of marking the reference axes in toric IOL implantation. It will include the very simple methods in low resource setting to the most advance and accurate ones in high volume centres. Finer nuances associated with each will also be discussed. Pitfalls of each method are also touched upon. Special focus is on manual marking methods as they are prone to human error. FF1-17 Dr Naren’s Fluid Pillar: A breakthrough technique for superior chamber stability and complication mitigation Naren SHETTY India Chamber stability is crucial in phacoemulsification to prevent surgical complications. We introduce a novel and simple technique that enhances chamber stability, quickens surge compensation and improves the performance of our currently existing phacoemulsification machines, allowing for operations at lower intraocular pressures (IOP) than currently possible. This video will explore the mechanics of this technique, its benefits—including improved safety and efficiency of current phaco machines—and its potential to significantly reduce the risk of surgical complications. FF1-18 My Way to Tackle Dislocation - Complex surgery easy approach Sugnesh PARMAR India In this video I have shown surgical management of various complicated & complex cases of dislocated IOL, IOL bag complex, nucleus due to various causes like zonular weakness, trauma, surgical complication etc. I have managed those cases with vitrectomy & posterior iris claw IOL implantation to give good post operative outcome. I want to show that Posterior Iris claw IOL implantation also give good outcomes in comparison with SFIOL.
