친애하는 동료 여러분,

연말을 맞아 지난 한 해를 돌아보면, 특히 생생하게 떠오르는 기억은 Ahmedabad 에서 성공적으로 개최된 제37회 APACRS 연례 학술대회입니다. 주제 선정이 매우 적절했고, “Learning from the Masters”는 제가 2025년에 참석한 교육 프로그램 가운데 가장 가치 있는 모임 중 하나였습니다. 이번 EyeWorld Asia Pacific에는 Thomas Kohnen의 훌륭한 Lim Lecture가 수록되어 있으며, 학술대회의 핵심 세션 일부도 함께 조명되고 있습니다. 아울러 이번 호에는 백내장 수술 후 최적의 결과를 얻기 위해 세심히 살펴야 할 세부 요소들에 초점을 맞춘 글들이 실려 있습니다. 그중 항상 주의를 요하는 한 가지는 안구표면의 건강입니다. 이는 시력에 영향을 줄 정도의 건성 상태 치료를 넘어, 눈물막의 비교적 경미한 문제까지도 포함되며, 이러한 문제는 술 후 굴절 결과 예측의 정확도에 영향을 줄 수 있습니다. 과거에는 안축장(axial length) 측정의 오차가 예기치 않은 굴절 결과의 주요 원인으로 오랫동안 간주되었습니다만, 현재는 상황이 달라졌습니다. 오늘날 예측 부정확성의 가장 큰 기반이 되는 지표는 각막곡률측정(keratometry)입니다. 과거 굴절수술이나 원추각막과 같은 구조적 변화로 인해 각막 불규칙성(irregular cornea)이 있는 경우에는 인공수정체(IOL) 도수 예측에 맞춤형 접근이 필요합니다. 그러나 눈물막 문제를 포함한 안구표면 최적화는 백내장 수술을 받는 모든 눈에 보편적으로 해당되는 과제입니다.
