FF1-25 The Inseparables! .....Conquering the tough posterior plate in rock-hard cataracts

Phacoemulsification of rock hard cataracts is difficult. Breaking the posterior plate of a Hard Cataract is a never-ending challenge and most complications happen during this stage of the surgery. This film highlights the difficulties faced during phacoemulsification of a hard cataract. We propose a new way to manage these tough cataracts. FF1-26 DUCK (Down Under and CounterclocKwise) Thanapong SOMKIJRUNGROJ Thailand DUCK, or Down Under and CounterclocKwise is a novel surgical technique designed to rotate and refine the position of a 1-piece C-loop toric IOL. A 1-piece C-loop toric IOL is a standard treatment for regular corneal astigmatism in patients undergoing cataract surgery. Thus, the design of the IOL generally requires positioning in the capsular bag with clockwise rotation intraoperatively. DUCK is a safe, easy, and efficient technique designed for counterclockwise rotation, used to refine the final position of the toric IOL. One must have the M-SICS technique in their armamentarium in case a complicated phacoemulsification surgery needs to be converted. It is also the primary procedure of choice, especially for hard cataracts, in high-volume set ups as they are economical and provide equally successful post-operative results. The video highlights snippets selected from cases, in a high-volume surgical centre, with hard mature cataracts and their nuclei being delivered using the viscoexpression technique.
