FF2-12 Managing Polar Cataract Bhavik ZALA India Posterior Polar Cataract are most of the time very tricky to manage. Every case is different when started and required proper pre surgery and intra operative planning for good outcome without any complications. FF2-13 Recognizing Subtle Signs of Zonular Compromise: The golden hour in decision making Madhu SHEKHAR India Zonulopathy and lens subluxation are frequently encountered in clinical practice. A thorough pre-operative evaluation often reveals obvious indicators of zonular compromise, such as gross lens subluxation, phacodonesis, iridodonesis, vitreous prolapse etc., enabling appropriate surgical planning. However, subtle signs are often overlooked, which can complicate management. This video highlights critical but less apparent signs, including the visibility of the lens equator following mydriatic injection, posterior chamber dye seepage, difficulty in initiating rhexis, anterior capsular dimpling, peripheral anterior lens capsule wrinkling, resistance during nucleus rotation, and anterior chamber fluctuations during phacoemulsification. Identifying these subtle cues in time—the "golden hour"—is pivotal for effective management and optimal surgical outcomes. FF2-14 Double CTR as Saviour in Large Subluxated Cataract With Microphthalmia and Irido-fundal Coloboma Amit RAJ India A 23-year-old male with decreased vision in both eye since childhood. Deteriorating of vision since 6 months in RE, while in LE was Pl -ve. There was bilateral inferior iris coloboma with total cataract with deficient zonules. AL was 29.09mm and 28.52mm and corneal diameter was 10mm in both meridia. After application of 4 iris hooks inferior suluxation was 7 clock hours. After capsulorrhexis, viscodelineation was done. After insertion of first CTR, phacoemulsification started with low parameters. Due to unstable bag another CTR with opposite orientation of eyelet was inserted. Aspiration of the remaining lens matter completed with viscoelastics. A 3 IOL implanted with haptic oriented in the weak area. Postoperatively well centred IOL was noted. FF2-15 Simplified Ab Externo Repair Technique for Large Iridodialysis Shilpa TARINI India Iridodialysis repair is often seen as challenging, requiring advanced skills and equipment. However, success lies in understanding the defect's anatomy and applying the right technique. A man in his late fifties with a brown cataract, small pupil and pseudoexfoliation developed a large inferior iridodialysis (2–10 o’clock) and zonular dialysis during cataract surgery, necessitating anterior vitrectomy and leaving him aphakic. Three months postoperatively, the 8-clock hour iridodialysis was repaired using an ab externo technique. This video demonstrates the technique involving minimal entry sites, use of an anterior chamber maintainer to ensure chamber stability and the anchoring of iris at four sites with McCannel sutures through reverse Hoffmann scleral pockets to bury the prolene suture knots. It highlights the simplicity in instrumentation and minimal manipulation involved in achieving the outcome. FF2-16 Beyond the Norm - PFV, coloboma and cataract Deeksha RANI India Join us for an insightful exploration into the surgical management of a complex congenital cataract case. FF2-17 Leaky Blinders: Novel use of the DSAEK triple procedure for strength and sight Rushad SHROFF India Patients with rheumatoid arthritis and corneal melts along with dense cataracts pose a therapeutic dilemma. Conventional keratoplasty and cataract surgery can not be performed due to the high risk of graft failure and melt. This video demonstrates the novel use of a DSAEK graft along with phacoemulsification to remove the cataract, provide internal tectonic support and replace the damaged endotheial cells in the same sitting. The stability of the corneal surface as well as favourable visual outcome with glasses and PROSE lenses indicates that this novel approach can be considered if faced with a similar challenging scenario in the future.
