FF2-18 Short and Long Eyes - Challenges of the extremes

This video is a compilation of seven cases, five with long eyes and two with small eyes illustrating various challenges faced during phacoemulsification and the techniques used to successfully tackle them. Among the long eyes, First case stresses the basic points to be taken care of in myopia. Second case demonstrates lens-iris diaphragm retropulsion syndrome. Third case was a very high myope (axial length 35.05mm) with posterior polar cataract. Fourth case discusses manifestations of IFIS in high myope. Fifth case deals with implantation of toric IOL in high myope. Among the short eyes first case was a small eye with iridochoroidal coloboma. Second case was a nanophthalmic eye where scleral windows were made to reduce the occurrence of choroidal effusion. FF2-20 Performing Safe Phacoemulsification in a Polar Cataract With a Pre-existing Posterior Capsular Dehiscence Suvira JAIN India This surgical video shows the management of a posterior polar cataract. What makes this case unique is the fact that the pre-existing dehiscence in the posterior capsule was quite obvious under clinical examination. This added an element of having to tread dangerous waters, and pulling it off successfully would require meticulous planning, suitable alteration in the surgical technique, a lot of skill and a little bit of luck! The surgical execution of this case will showcase the successful management of the polar cataract with a 3 piece IOL implantation without encountering vitreous disturbance or a nucleus drop. FF2-21 The Most Chivalrous Surgeon in the World Ranjan DUTTA India One of the challenging situations in cataract surgery is operating on a patient who is unable to lie supine on the operating table. We describe a novel approach wherein the patient sat on the surgeon's chair with the back of the chair abutting the headrest of the operating table. The patient's head was extended backwards resting on the table headrest akin to the position adopted while receiving a shampoo in a salon chair. Topical anesthesia with mild sedation was administered. Cleaning with Povidone solution and draping was done in the standard manner. Surgeon was seated temporal to the patient's eye on another chair. Uneventful phacoemulsification and IOL implantation was performed as Capsulorhexis is a very important step to perform successful phacoemulsification surgery. Zepto nano-pulse precision capsulotomy (PPC) is a novel capsulotomy method and technology. It is very useful in Premium IOL implantation cases for perfect centration of Trifocal and Toric IOLs. It is also helpful in difficult cases like white cataracts, Brunescent cataracts, Zonulopathy, small pupil and infantile cataracts, where an inadequate capsulorhexis may preclude the optimal outcome. Potential intraoperative problems encountered while using this device include inability to open the device, suction loss, incomplete CCC and difficulty in folding the device. Our purpose is to highlight iris suction, an uncommon intraoperative Zepto complication in routine phacoemulsification cases.
